Appointments

It would be our pleasure to serve you! Please contact our dental office either by phone and text at 612.926.9000 for the most convenient and expedient scheduling.  You may also email or fill out this appointment request and we will get back to you at our earliest convenience. Per WLDC privacy policy, consumer information is not shared with third parties for marketing purposes. 


Please do not use this form to cancel or change an existing appointment. We kindly request that you call us by phone to make schedule changes.

Available appointments during our office hours are Monday through Thursday, 7 AM to 4 PM.  We are closed for lunch from 11 AM to 12 PM.


Patient Forms

If you are a new patient to our office, please click this link which contains our new patient packet with forms that will need to be filled out when you arrive at our office. Thank you and please call our office if you have any questions. 

If you are an existing patient, please click this link to update your medical history and other necessary forms.

To release your records and xrays from your previous office, they may require you to fill out their preferred form.  If you would like, you can try filling out our form online here and we can request on your behalf. It may be best to contact your previous office directly, however. 


Payment Options

Credit Cards

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.

Dental Insurance

As a service to our patients, our practice accepts most dental insurance programs, including non-managed care, indemnity (traditional) and PPO out-of-network.  We are not part of any managed care network.  Our accounting staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be.

The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.
